Each town clerk shall receive or obtain and record facts relative to births in his town (MGLA c 46, s 1). Additionally, a parent of a child born outside the Commonwealth may present to the clerk of the town where domiciled at time of birth, an original certificate or other written evidence of birth. The clerk may file such documents (s 1B) and prepare a Return of birth outside of the Commonwealth (currently Registry of Vital Records and Statistics form R-4, per s 16), with copy to registry (s 17).
